If you don’t have a consent form, creating one yourself or consulting a professional is a viable path. The form, at the very least, must include the following:

  • Current Date: The exact date of the document’s creation.

  • Performer Details: Full legal name, date of birth, and full years of age.

  • Explicit Consents:

    • To be depicted in the content.

    • To allow public distribution and uploading of the content to the website.

    • To allow the content to be downloaded (if applicable).

An example of a written and executed consent form could read:

“I, [Mary Clair Austin], aged [23], born on [1 August 1998], consent to be depicted in the video(s)/image(s) created today with co-performer [Full Name]. I grant consent for the public distribution of videos/images through [YOURWEBSITEURL] and permit the content to be downloaded, should co-performer [Full Name] decide to allow users to do so.
