A Consent form, or a Model release, is a form of consent proof to supply your content with if performers other than the account owner are participating in it. Please feel free to refer to this guide to check whether the consent forms you use are compliant with ModelCentro TOS. If you don't have your own consent form, you will find our template available for download attached to this article, as well as advice on what your consent form needs to have if you are planning to craft your own.
Where can I download the template?
Please find our consent form template attached to this article. You can also download it from your model admin at any time by using the "Get Template" option when adding a guest model to your content.
I have my own template - is it compliant with ModelCentro TOS?
We will be glad to accept your own model release template as well. To make sure that it meets the approval requirements, your form needs to contain the following:
Current Date: The date when the document was filled.
Performer Details: Full legal name and date of birth.
Content Production Date: The date when the content was produced.
Explicit Consents:
Consent to be depicted in the content item.
Consent to allow public distribution of the content.
Consent to upload the content to [YOURWEBSITEURL].
Consent to enable content downloading (if applicable).
Signature: The performer's authentic signature.
IMPORTANT: Some model release forms you may find online may specifically exclude adult and pornographic content. If you are using a pre-made template and specialise in 18+ content, please make sure the release form you are using does not exclude it.
I want to craft my own model releases - what do I include?
If you don’t have a consent form, but need one for your brand, creating one yourself or consulting a professional is a viable path. The form, at the very least, must include the following:
Current Date: The date of the document’s creation.
Performer Details: Full legal name, date of birth, and full years of age.
Explicit Consents:
To be depicted in the content.
To allow public distribution and uploading of the content to your website.
To allow the content to be downloaded (if applicable).
An example of a written and executed consent form could read:
“I, [Mary Clair Austin], aged [28], born on [1 August 1998], consent to be depicted in the video(s)/image(s) created today with co-performer [Full Name]. I grant consent for the public distribution of videos/images through [YOURWEBSITEURL] and permit the content to be downloaded, should co-performer [Full Name] decide to allow users to do so.
[October 15th, 2023]
[Signature] Mary Claire Austin"
IMPORTANT LEGAL NOTE:
Creating a document in this manner is aimed solely at documenting consent to comply with Credit Card Association requirements and does not grant any rights regarding the content between the involved parties. To safeguard your rights pertaining to the content, professional legal counsel is advisable. If you require an attorney, assistance from adult law specialists is worth looking into.
